Submitting a game report
After every game you officiate, CHSAA asks you to file a game report. This page walks through getting access, filling out the form, and what you can expect after you submit.
Before you start
You need an officials account to submit reports. If you have already set a password for Officials Center (from your pass email or a sport director access email), you already have one — just sign in.
If you have never set a password, go to officials.chsaa.org/game-reports and enter the email the CHSAA office has for you. We will send you a link. The link expires in 72 hours, so use it when it arrives.
No account with that email? The game report portal uses the same email address as your officials registration. If the link does not arrive, check Spam / Junk, then contact the CHSAA officials office to confirm the address they have on file.
Sign in and open the portal
- Go to officials.chsaa.org.
- Tap Officials (not Association).
- Tap Sign in as an official and use your email and password.
- In the sidebar, tap Game Reports.
You will see a grid of the sports you are currently registered in. Only sports with a current dues year appear.
Choose a sport and open the form
Tap the sport the game was played in. The form opens for that sport.
Each form has several sections. You fill them in order, top to bottom.
Fill out the form
Game Details
Date, start time, site / location, and level (Varsity, Sub-varsity, etc.) are required. Some sports also ask for end time, game length, or a meet / event name.
Site — type the field or gym name and the city, for example Broomfield High School, Broomfield.
Teams
For most sports, you enter a Home team and a Visiting team. For meet sports (cross country, track, etc.), enter the host school and any participating teams relevant to the report.
Pick the school from the dropdown if it appears. If the school is not in the list, type the team name in the free-text box.
Scores — enter a final score if the form asks for it. You can leave it blank if you do not know.
Officials
The form pre-fills your name if you are signed in. Confirm it is correct. Add other crew members (by position) if the sport uses a multi-person crew.
You can look up a co-official by name — if they are in the CHSAA directory, their contact information links up automatically.
Ejections and sanctions
If there were no ejections or other sanctions, leave this section empty and continue.
If someone was ejected or received a card / conduct sanction:
- Tap + Add ejection (or + Add sanction / card for sports with card vocabulary).
- Choose who was sanctioned: Player, Coach, Admin, Bench Personnel, Spectator, or Other.
- Choose the school. This is how the ejection letter finds the athletic director.
- Fill in the person's name. For players, add their jersey number.
- Choose the foul or card type (the options depend on the sport).
- Add the period / quarter / half and any time remaining.
- Reason is required. Write a brief, factual description of what happened.
Repeat for each person sanctioned. You can remove a row with the ✕ button.
Ejection letters: when you record an ejection, the CHSAA officials office and the school's athletic director receive an automatic email.
Game Management
Check any box where you have a concern to report and add a brief description:
| Box | What it covers |
|---|---|
| Event management | Pre-game setup, host communication, timing |
| Staffing | Chain crew, scoreboard, or other game-day personnel |
| Facilities | Field / court / ice conditions, locker rooms, lighting |
Leave the boxes unchecked if there is nothing to report. A concern in any of these sections notifies the area director and sport staff — it does not block submission.
Sportsmanship
One row appears for each team. Check Sportsmanship concern if a team's conduct was a problem and describe it briefly.
Additional Comments
Free-text for anything else you want the office to know. This is optional.
Submit
Tap Submit report at the bottom of the form.
If something is missing, the page will scroll to the first problem and highlight it. Fix the issue and try again.
When it succeeds, you will see a Report submitted confirmation page and receive a confirmation email.
After you submit
Confirmation email — sent to your email address immediately. It includes a link to your report and a summary of whether sanctions or concerns were flagged.
Area director and sport staff notifications — if you recorded a sanction or a concern, the relevant area director and sport office contacts receive a notification email.
Ejection letters — if you recorded an ejection, the school's athletic director receives an automatic letter by email.
No response needed — unless the office reaches out, there is nothing else to do after submitting.
View your past reports
From Game Reports in the sidebar, your ten most recent submissions appear at the bottom of the page. Tap View all to see your full history.
If something goes wrong
I do not see the sport I need. You can only submit for sports you are currently registered in (dues paid for that sport's current year). If a sport is missing, contact the office — they may need to update your dues record.
I made a mistake on a submitted report. Contact the CHSAA officials office. Staff can view and correct submitted reports.
The form will not submit. Read the red message near the bottom. Common causes: Reason is missing on a sanction, a required field is blank, or the date is outside of an expected range.
I got the access email but the link is expired. Go to officials.chsaa.org → Officials → Forgot your password? to get a fresh link. Or contact the office.
Need help?
Contact the CHSAA officials office. They can look up your record, check whether a report was received, and correct mistakes after submission.
